WHAT TO BRING
- The referral from your GP or specialist
- Any relevant previous imaging (if you don’t have these, please let our staff know what company you had your imaging with so we can access this online)
- Any relevant orthotics
- Your completed registration form – this will be forwarded to you with an appointment confirmation letter prior to your consultation. Alternatively you can download this form here.
IF YOU NEED TO CANCEL YOUR APPOINTMENT:
Please call our office on 08 9200 6052 during business hours and please provide at least one days notice so we can offer the appointment opportunity to another patient.
We recognise your time is valuable and we make every effort to run on time. Occasionally emergencies or patients require a little more time and these cause scheduling delays beyond our control. We apologise if we keep you waiting.
